10/1/2023 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a cheerful mood evident by her overall disposition. QMHPA inquired about her afternoon. Ms. Broadnax verbalized to QMHPA that she was having a good afternoon so far and that she woke up around 5:00am and began cleaning her home and writing out her list of things that she needs in her home. QMHPA informed Ms. Broadnax of the goal and objective for this session. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Ms. Broadnax’ current thoughts, feelings and emotions. QMHPA engaged Ms. Broadnax in discussion surrounding her mental health. QMHPA engaged Ms. Broadnax in discussion surrounding a community activity. QMHPA informed Ms. Broadnax that the purpose of this activity was for Ms. Broadnax to gain resources in an effort to save money in reference to monthly grocery/household expenses. QMHPA reviewed activity with Ms. Broadnax. QMHPA prompted Ms. Broadnax to obtain itemized sheet produced during previous session. QMHPA reviewed the itemized sheet with Ms. Broadnax. QMHPA engaged Ms. Broadnax in a skill building activity. QMHPA informed Ms. Broadnax that the purpose of this activity was for Ms. Broadnax to understand how to budget her household needs and any other expenses within the home. QMHPA introduced couponing in an effort to assist Ms. Broadnax with saving money. Towards the end of the session QMHPA provided Ms. Broadnax with positive reinforcement and praise she completed the activities. Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax discussed with QMHPA that she was in a good mood when discussing her mental health. During the skills building activity, Ms. Broadnax was able to provide QMHPA with what she needs to obtain in her home and what she doesn’t in an effort to begin saving money. Ms. Broadnax discussed with QMHPA that she had written out a budget sheet that QMHPA asked her to provide her with in a previous session. Ms. Broadnax acknowledged brand vs. non brand and indicated to QMHPA that she is going to learn how to shop for things this not costly. During the community activity, Ms. Broadnax has clipped coupons to assist her with budgeting her money for the month. Ms. Broadnax indicated to QMHPA that she thinks that she is going to like the couponing, but she is going to have a problem with being consistent.
